Patient Forms
Practice Policies
Billing
We accept checks, cash, debit and all major credit cards including Visa, Master Card, American Express and Discover. We require payment of your past due account balances and co-payments at time of service.
Insurance
We submit all claims to most major insurance carriers. Please present your current insurance card and ID at every visit so we can verify all records are updated. Please notify us if you have any changes to your name, address, phone number and insurance. Please call your insurance company to verify that Frank Thomas Dean, MD is in network with your insurance carrier.
FMLA/Disability Forms
All forms required for your work will be reviewed on a case by case basis. The forms can be hand delivered to the office or faxed to 972-298-8903. We will need 7-14 days to complete the forms and will notify you by phone when forms are ready for pick up. Please ensure we have a current phone number to contact you. There may be a $25 fee for completion.
Reschedule or Cancel appointment
We request a 24 hour notice for reschedule or cancel of appointments and charge a $25 fee for no show appointments.
